Please note:
- Missouri pharmacies: Individuals must be 12 years of age or older. Parental consent will be required for patients 12 to 17 years of age and parent/guardian must be present.
- Illinois pharmacies: Individuals must be 14 years of age or older. Parental consent will be required for patients 14 to 17 years of age and parent/guardian must be present.
- Please dress appropriately. Access to the upper arm, without disrobing, is required for administration of vaccine.
